Voter Registration Forms
Find application forms to update, add or remove your information from the voters list, Register of Absentee Voters or the Ontario Register of Future Voters.
You must include one accepted piece of ID showing both name and address to update, add or remove your information. To notify Elections Ontario of a death, you must include a document showing proof of death.
You can complete any application form on this page and return it to us by:
- email to email@example.com with an attached copy of your ID or documentation; or
- mail with and a photocopy of one piece of ID or documentation.
You can also use eRegistration to confirm, update or add your information.
