Voter Registration Forms

Find application forms to add, update or remove your information from the voters list, Register of Absentee Voters or the Ontario Register of Future Voters

You must include one acceptable piece of ID showing both name and address to add, update or remove your information. To notify Elections Ontario of a death, you must include a document showing proof of death.

You can complete any application form on this page and return it to us by:

  • email to register@elections.on.ca with an attached copy of your ID or documentation; or
  • mail with and a photocopy of one piece of ID or documentation.

You can also use e-Registration to add, update or confirm your information online.
