By filling out this online form, electors can query the permanent register of electors to see if their information is contained in the permanent register. The permanent register is used to create the Preliminary List of Electors.

To ensure that Elections Ontario maintains the privacy of the permanent register and in order to verify an elector’s personal information, we require the submission of an elector’s complete first name, last name, date of birth and residential address.
